Sage Behavior Services contracts with a company called CodeMetro for their NPAWorks software. This software is used for our scheduling, billing, and HR purposes, and as such is used to store various forms of personal information regarding our clients and employees. On the evening of June 1st, Sage received notice from CodeMetro that they suffered a “data security incident” which resulted in the potential breach of sensitive data. Specifically, unauthorized individuals were able to access a CodeMetro database server which contained health-related client information and employee payroll information from the companies for which they provide services. Below is a blank version of the breach notice letter that CodeMetro attempted to send to all families affected by their breach.
